  • Paperwork

    Please click on the links below to access all paperwork you'll need throughout treatment. The "informed consent" form is to be completed prior to the first session. The "release of information" form should only be completed if requested.

    One person can complete the "demographic information" section of the informed consent for everyone participating in therapy, though everyone will need to read and sign the actual informed consent. 

    Please contact me for the password needed to access each of the forms. Each form takes about 15 minutes to complete. The digital forms are stored on secure, HIPPA-compliant servers. If you prefer to fill out a paper/pencil version prior to the first session, let me know.

  • Availability

    I see clients on Wednesday from 9:00 am to 5:00 pm. Some exceptions to this schedule are made as my academic schedule allows.

  • Fees & Insurance

    Session fees are $170 for a 50-minute session. Payment can be made at the beginning of the session via cash, check, or card.

    I do not take insurance, though many PPOs will reimburse you for payment. Let me know if you'd like to submit a reimbursement request to your PPO and I'll provide you the necessary paperwork. Also, most Health Savings Account (HSA) plans can be used to pay for my services.

  • Cancellation Policy

    Out of respect for those on the waiting list, cancellations made less than 24 hours in advance will be billed at the full session rate.
